User account

Physicians and Medical Managers with a membership in McHenry County Medical Society have had user accounts already created.  Please don't create a new account, we will be sending your login information shortly.

In order to create an account at McHenry County Medical Society you must provide a username and a unique email address for your account.  If you use an email already in our system it will be rejected as a duplicate.  You may apply for a role as either a medical manager, physician, or vendor, which will be approved by an administrator.

Account information
Spaces are allowed; punctuation is not allowed except for periods, hyphens, and underscores.
A valid e-mail address. All e-mails from the system will be sent to this address. The e-mail address is not made public and will only be used if you wish to receive a new password or wish to receive certain news or notifications by e-mail.
Provide a password for the new account in both fields.
